In 2013 Masahiro Ito was commisioned to work on a Silent Hill sequel.[1] As Ito always conceived Pyramid Head as a personal James Sunderland's illusion, he never wished to use him again in another Silent Hill game.[2] [3] Understanding, however, that it would be hard to convince Konami to create a new Silent Hill title without this iconic creature due to Pyramid Head's immense popularity, for this project Ito tried to "get a new impressive direction which users accept" and create new versions of Pyramid Head and Valtiel that would be distinct from previous games. In this new plan Valtiel would kill Pyramid Head in the opening scene.[4] [5] [6]

Masahiro Ito couldn't reveal any specifics about this canceled Silent Hill and only posted one low-res image depicting what seems to be a baby carriage in the Otherworld and named it "the beginning of its story".
